What is a Careplan?

 

Careplans or plans for care for seniors is the topic of this webpage.  In  working as a nurse for a long time and have seen both good and bad care for seniors. I think they are one of the most vulnerable groups who access healthcare. I want to demonstrate what is involved in creating a plan of care for the nurses, doctors, health care assistants and other healthcare professionals in the hospital.

A careplan should be individualized to each person. That is 'best practice' and changes the care-giving from a task orientation to a person-centred orientation. I have included links at the top for instructions, case studies and"what you should know about me" for Martha.
