Directions for Creating a Careplan

 

using the Kwantlen Needs

 

 

We are going to look at the information you need to assess in order to create a unique, thoughtful care plan for your resident. This is a skill you need to develop in order to provide professional, compassionate care-giving to your residents.

 

The first thing you need to do is look at the Needs of your patients according to the Kwantlen standards.

 

The needs are:

 

  • Protection and Safety-focus on infections, skin care and pain control

  •  

  • Nutrition-focus on appropriate diet and texture for the individual; look at swallowing ability 

  •  

  • Elimination-focus on constipation and continence or the ability to manage bladder/bowel control

  •  

  • Sensory Stimulation-focus on the senses sight, hearing, touch, smell and taste

  •  

  • Rest and Activity-focus on falls and abilities to take part in activities

  •  

  • Oxygen-focus on the respiratory system, circulatory system, strokes, and heart attacks and other circulatory health

  •  

  • Life fulfilment-focus on sexual health, palliative care and death and dying and supporting residents and families through this.

 

 

 

 

Reportable Observations

 

The following is a list of items that can be assessed:

 

  • is the person hard or easy to wake up?

  • does the person identify people properly?

  • can the person squeeze your fingers with each hand?

  • does the person complain of sore joints?

  • is the skin pale or flushed?

  • does the person like the food that is served?

  • is there adequate urine production?

  • is the abdomen firm or soft?

  • do they have bowel sounds?

  • how does the person rate on the pain scale if 0 is none and 5 is severe?

  • does the person complain of spots before their eyes, flashes or or blurring?

  • do gay seniors fear being treated equally in the hospital?

  • is there drainage from the ears?
